Terms of Service

These Terms govern your use of this website and the immigration medical examination services NY MedCare provides in Manhattan. The immigration medical examination is an unusual kind of medical visit, because it exists to complete a government form. Sections 3, 4, and 8 explain what we can and cannot do for you, and are worth reading before your appointment.

Section 1Agreement to these Terms

By using this website, calling to book an appointment, or receiving services from us, you agree to these Terms of Service and to our Privacy Policy. If you do not agree, please do not use the site or our services. These Terms are between you and NY MedCare, 15 E 40th St, Suite 201, New York, NY 10016.

Section 2What we are, and what we are not

Dr. Huma Irshad is a physician licensed in New York and a USCIS-designated civil surgeon, meaning USCIS has authorized her to perform Form I-693 immigration medical examinations and certify the results. You can verify any civil surgeon's designation with the USCIS Find a Doctor tool at my.uscis.gov/findadoctor.

Section 3The nature of the examination

The Form I-693 examination is a screening examination performed for USCIS. Its scope is set by USCIS and by the CDC Technical Instructions for Civil Surgeons. That has real consequences:

• It is not a complete physical and it is not primary care. It will not diagnose everything and is not a substitute for a relationship with a primary care physician.
• We examine and document; we generally do not treat. If the examination finds a condition we will tell you and refer you. Treatment is separate from the immigration examination and is not included in the exam fee.
• The required tests are not optional. We cannot omit a test, a vaccine, or a question because you would prefer we did. A form completed that way is not a valid form.
• We must record what we find. Dr. Irshad certifies the form under her own license and signature and cannot certify findings she has not made or knows to be inaccurate.

Section 4No guarantee of any immigration outcome

We complete your examination accurately and on time. What USCIS does with it is outside our control.

• We cannot promise your application will be approved, or that it will move faster.
• We cannot promise USCIS will accept a form, waive a requirement, or decline to issue a Request for Evidence.
• If the examination identifies a Class A condition, meaning certain communicable diseases of public health significance, a physical or mental disorder with associated harmful behavior, or drug abuse or addiction as defined by the Technical Instructions, we are required to document it and it may affect your admissibility. We cannot leave it off the form. Discuss consequences and any waiver options with your attorney.
• Paying our fee purchases a professionally performed examination and a properly completed form. It does not purchase a result.

Section 5Appointments and cancellations

• Appointments are made by telephone at (917) 905-8140. This website does not take bookings.
• Please give us at least 24 hours' notice to cancel or reschedule. Repeated missed appointments without notice may result in a fee or in our declining to reschedule.
• Arrive on time and bring everything listed in Section 6. If required documents are missing we may not be able to complete the examination that day.
• We may need to reschedule for illness, emergency, or circumstances beyond our control. If we do, we will offer the earliest available alternative.

Section 6Your responsibilities

To complete your examination, you agree to:

• Bring valid government-issued photo identification
• Bring your vaccination records, every record you have, from any country. Missing records usually mean repeating vaccines or paying for blood titers.
• Bring Form I-693, current edition, with Part 1 completed by you and left unsigned until Dr. Irshad instructs you
• Bring your USCIS receipt notice or A-number if you have one
• Tell us about relevant medical history, medications, prior tuberculosis testing or treatment, and any prior immigration medical examination
• Give complete and truthful information. Providing false information to a civil surgeon can invalidate your form and carries serious consequences for your immigration case. We may decline to complete an examination where we cannot verify identity or where we are asked to record something inaccurate.
• Complete the laboratory tests and vaccines we order promptly, so results can be recorded

Section 7Fees and payment

• The examination fee is quoted before your appointment and is due at the time of service.
• Laboratory tests, vaccines, chest radiographs, and any treatment are billed separately, either by us or directly by the laboratory, radiology facility, or pharmacy providing them.
• Health insurance generally does not cover immigration medical examinations, because they are performed for a government form rather than for medical necessity. Some insurers may cover vaccines. Coverage is between you and your insurer and you remain responsible for the balance.
• Once the examination has been performed the fee for it is earned and is not refundable, regardless of the outcome of your immigration case.
• Fees for copies of records are limited to what New York law permits.

Section 8Your sealed envelope and your form

• We complete, sign, and seal your form once all laboratory results and required vaccinations are in.
• You are responsible for submitting the sealed envelope to USCIS. We do not file your immigration application and do not send the form to USCIS for you.
• Form I-693 must be signed within specific time windows relative to your filing, and USCIS periodically changes both the accepted edition of the form and its validity rules. Confirm current requirements at uscis.gov/i-693 or with your attorney before you file.
• If you lose the envelope, or USCIS requests a replacement, we can usually reissue from your record. A reissue may involve a fee, and if too much time has passed a new examination may be required.

Section 9Telephone and text messages

If you give us your mobile number and agree, NY MedCare may send appointment confirmations, reminders, and results-ready notices by text message.

• Consent. By giving us your mobile number and opting in you authorize us to text that number. Consent is not a condition of purchase or of receiving care.
• Opt out. Text STOP to any message at any time. We will send one confirmation and then stop. To rejoin, opt in again the same way.
• Help. Reply HELP, email info@nymedcare.com, or call (917) 905-8140.
• Frequency and cost. Message frequency varies. Message and data rates may apply; check with your wireless provider.
• Carriers. Wireless carriers are not liable for delayed or undelivered messages.
• Privacy. No mobile information will be shared with third parties or affiliates for marketing or promotional purposes. Sharing with subcontractors that deliver messages on our behalf is permitted. See our Privacy Policy.
• Not for emergencies. Text and voicemail are not monitored around the clock. For a medical emergency call 911.

Section 10Use of this website

The content on this site, including text, layout, graphics, logos, and photographs, belongs to NY MedCare or its licensors and is protected by copyright and trademark law. You may read, print, and share pages for personal, non-commercial use. You may not:

• Copy or republish substantial portions of the site commercially
• Scrape, crawl, or harvest the site by automated means beyond ordinary search engine indexing
• Attempt to gain unauthorized access to our systems or interfere with the site's operation
• Use the site in a way that violates any applicable law

Section 11Information on this site is general

The pages describing the I-693 process, required vaccines, what to bring, and typical timelines are general information, current as of the date shown, offered to help you prepare. They are not medical advice for your situation and not legal advice about your case. USCIS requirements and the CDC Technical Instructions change, sometimes with little notice. Always verify current requirements with USCIS or your attorney, and rely on Dr. Irshad's direct guidance for your own examination.

Section 12Third-party services and links

The chat window on this site is provided by LeadConnector (HighLevel) and loads from their servers; what you type into it is stored by them on our behalf, and it is not a secure or continuously monitored channel — do not send clinical detail through it, and call 911 in an emergency. This site also loads fonts and a graphics library from third-party content delivery networks and uses Google Analytics for aggregate measurement. We also link to USCIS, the CDC, and other outside resources. We do not control those services or sites and are not responsible for their content, availability, or privacy practices. Your use of them is governed by their own terms.

Section 13Disclaimers and limitation of liability

This website is provided "as is" and "as available." We do not warrant it will be uninterrupted, error-free, or free of harmful components, and we disclaim implied warranties of merchantability, fitness for a particular purpose, and non-infringement to the fullest extent the law allows.

To the fullest extent permitted by law, NY MedCare is not liable for indirect, incidental, consequential, special, or punitive damages arising from your use of this website, including damages related to delay, denial, or any other outcome of an immigration application. Our total liability for any claim arising out of the website or the scheduling of an appointment is limited to the amount you paid us for the service at issue.

Section 14Emergencies

This website and our telephone line are not emergency services and are not monitored continuously. If you are experiencing a medical emergency, call 911 or go to the nearest emergency department.

Section 15Governing law

These Terms are governed by the laws of the State of New York, without regard to conflict-of-laws principles. Any dispute not resolved informally will be brought in the state or federal courts located in New York County, New York, and you and NY MedCare consent to venue there.

Section 16Changes, severability, and contact

We may update these Terms as our services or the law change. The current version is always the one posted here, with its effective date. If any provision is held unenforceable the remaining provisions stay in force. Our failure to enforce a provision is not a waiver of it.
